According to (Oermann & Gaberson, (2016), assessment is appraisal while evaluation is the determination of the value of a product or service. Assessing involves understanding the state and conditions of items through observation and measurement. On the centrally, evaluation involve the observation and measurement of products and services with the aim of judging and determine the value in comparison to a given standard. Assessment means the obtaining of information with a specific purpose. Information collected may be either qualitative or quantitative based on the purpose of assessment. The main aim of assessment is the understanding and improvement of student learning (Rashvand et al., 2017). Evaluation, on the other hand, is commonly associated with the summed evaluation taking an assessment on the higher level where the quality of performance is defined. The summative evaluation will, therefore, mean that there can be a decision-making process.

The assessment in the clinic involves aspects such as effectiveness that will be measured on the basis of the improvements in the healthcare provision, efficiency that relates to the benefits obtained, and acceptability in the social and psychological treatment of people in the provision of care. Another assessment occurs in the fair distribution of services in the healthcare provision. An evaluation in the clinic can be done during an intervention in healthcare so that the results of the information on the ongoing progress (Rashvand et al., 2017). The evaluation here can be done either prospectively or retrospectively.

On a prospective basis, there is an advantage in ensuring that the data collection is planned adequately that only specific questions are posed. Retrospective aspect dredges on the proxy indicators and is built on the intrinsic part of services or projects (Rashvand et al., 2017). Assessment is the process of appraisal for something or even someone, gauging the quality and value. On the other hand, evaluation is the process of making judgments on the values, numbers, and the performance of a person or something (Chae et al., 2018). This difference lies in the orientation as the assessment is process-oriented while evaluation is product oriented. Evaluation in the clinic setting is designed in a way to document the level of achievement which has been attained. On the other hand, the assessment focuses on the measurement of performance, the work product, or the skills so as to offer feedback on the service strength and weaknesses and give the direction where improvement is needed.

While in assessment, there is the use of dynamic and not static ways of collection and analysis of the clients, which is the first step in the delivery of nursing care, evaluation, on the other hand, is done on both patient status as well as the effectiveness of the nursing care leading to the modification of the care plan as needed. The analysis includes the use of psychological data, social-cultural, spiritual, economic, as well as lifestyle factors (Oermann & Gaberson, 2016). For instance, the nurse assessment of clients who are hospitalized in pain includes the physical causes as well as the manifestation of pain and response of the patient, such as the inability to get out of the bed and refusal of eating and many more. Evaluation is the measurement of the degree with which the goals are achieved and the positive and negative factors that have influenced the achievement of the goal (Rashvand et al., 2017). This determines whether to continue, modify, and terminate the care plan.
