There is a wide range of findings that can be identified in the self-management of diabetes. However, it is important to note that there is a need for a counselor to manage the psychological aspects of diabetes in patients. Therefore, this is an indicator of all the diabetic programs to embrace the self-management plan in managing diabetes. It better improves patient outcomes and is cost-effective.

Challenges Experience

Regardless of the clear outcomes and findings presented in the project, I face several limitations. It was quite challenging dealing with the overstretching family responsibility of the patient and the cost or financial challenges linked to managing diabetes. Additionally, finding committed clinicians also proved to be challenging regardless of having the facility's full support. However, it is necessary to note that this was paramount for improved results. However, the primary challenge that proved to be detrimental to the project was the lack of adequate staff, and this is because it was important to engage patients in education while at the same time managing the diabetic plan for the rest of the patients. This is a clear indication that the facility needs to recruit additional staff.

Initial Research

The initial research or study was a building foundation for the project. Through research, it was able to assess the prevalence of diabetes internationally. It was determined that without proper guidance, so many individuals succumb to this ailment. Therefore, this was a call to implement effective interventions that change the status quo and make a positive individual in 2020. Therefore, self-management was the first step towards the achievement of this objective. Additionally, the initial research made it clear why a multidisciplinary approach is important in formulating possible solutions and the vital role of healthcare nurses in diabetes management.

Employing the self-management plan in the health care facility serves as the initial fist plan on a long journey in diabetes management. A follow-up with the patient in the facility was done weekly, which was done by employing technological devices and systems. The outcome of the self-management in diabetes showed increased confidence among individuals living with diabetes. It showed positivity after the first session and a lot of enthusiasm for the rest of the program. Based on past research and implementation of the plan, there is a high chance of program success and yield positive results in patient satisfaction, reduced financial burden, decreased AIC levels, and better patient outcomes (Cunningham et al., 2015). However, this is feasible if the patients adhere to the programs and follow the tips included in the educational materials they are presented with. The results recorded after the first session we tremendous, and they surpassed my expectations. And this stirred the assumption as to diabetes self-management and education were the missing link in assisting patients in dealing with diabetes

Health Information and Patient Care Technology

A follow-up with the patient every two weeks can be of great importance in helping patients deal with their diabetes. It excites the patients to follow up processes by their doctors, which is done through messaging, phone calls, and medical apps. Additionally, technology played a vital part in the communication if the educational materials linked ideas of the different collaborators through the emailing system. Therefore, technology plays a vital part in the formulation of the product and the execution of the end product of the professional output (Lucas, 2015). Additionally, remote face-to-face with the patient was done in addition to the use of brochures.

Impact of Organization and Health Care Policies

The organization was very positive about the program as it was perceived to improve patient care. Consequently, this was the first step that the organization took in engaging Medicare and Medicaid reimbursement for their diabetic patients in the facility. Through proper financing, the program was successfully implemented.

How the Project Helped Me Grow

Through the entire project, I have acquired vast knowledge of the prevalence of diabetes, its progression, factors that contribute to diabetes, signs and symptoms of the ailment, and the impact it has on the patient, community, and family. I have also learned that technology can impact all healthcare sectors. Its proper utilization can be a critical tool in the management of diabetes as it enhances connectivity between patients and their care providers. This project has allowed me to employ my theoretical skills in a real-life case setting, which has been a major part of my development.

It is also important to note that it aligns perfectly with the nursing code of conduct. Showing an enthusiastic approach to the program encouraged me to involve more patients in the program, creating a new path towards bettering the lives of people living with diabetes and the entire community. However, if given another opportunity to undertake this project, then I would certainly engage a psychologist as a collaborator. All in all, this was a successful program that empowered the patient to engage in diabetes self-management as an alternative care intervention.
