As an expert, there are fundamental questions that should have been asked to understand better the health problem facing the patient. Jack Baldwin is a 50-year-old male with health conditions that relate to hypertension. One of the questions that should have been asked is the social relationship of the patient. Even though the patient is monogamous, it was important to better understand the relationship of the patient with close family members (Dains, Baumann, & Scheibel, 2015). The question would be important in understanding factors that could put the health of the patient in jeopardy. Besides, a thorough assessment of the lifestyle of the patient, including the choice of foods taken and level of exercise is crucial for a better diagnosis.
Physical examination is critical in establishing the problem facing the patient. In the case of Jack Baldwin, the physical examination failed to examine the blood pressure of the patient when exposed to different physical activities. Besides, the examination did not measure respiratory rates in a manner that could establish how the patient breathes and the impact on potential high blood pressure disease (Peitzman & Cuddy, 2015). These are the two errors that the health officer in charge failed to conduct to improve the process of diagnosis for this patient. HPB is a health problem that puts the patient in danger under different circumstances, and the examination would have provided the right indices for assessing the health issue.
The key finding that I included in the physical exam is the weight of the patient. There is a direct link between the weight of the patient and the development of high blood pressure. High blood pressure is a lifestyle disease that is linked to overweight. There is a need to establish the BMI of the patient to determine if his weight puts him at much risk for high blood pressure (Peitzman & Cuddy, 2015). Besides, the examination will also help in designing a treatment plan that will allow the patient to improve health outcomes to reduce the risk of suffering from HPB. Management of weight is crucial in the management of HPB.
I believe that the breast examination, in this case, is incorrect based on the primary diagnosis of this patient. The examination to establish if there is some form of discharge does not relate in any way with HPB. The examination would have been appropriate for a female patient who was under the examination for breast cancer (Peitzman & Cuddy, 2015). Even if the patient is at higher risk of developing cancer, the history shows that prostate cancer lies in his family history. Besides, siblings have no history of cancer, making this examination inappropriate for this diagnosis. Nonetheless, the examination is part of the clinical procedure, which must be followed by medical practitioners.
The differential diagnosis I missed in the examination is the previous medication for this health issue. The patient in question has been ha had a lifestyle that puts him at risk of lifestyle diseases. The previous examination of this disease would be pivotal in identifying the extent of spread or the risk of the patient in suffering further from the health issue (Dains, Baumann, & Scheibel, 2015). The two diagnoses conducted on the patient targeted prostate cancer ad HPB, but it is evident that the patient is at much risk of developing HPB. The differential diagnosis will help to establish further if the patient is suffering from HPB and help develop a treatment plan that would improve the quality of life of the patient.
