From the provided case study, the most probable axis 1 diagnosis is disorganized Schizophrenia. There are various symptoms exhibited by Adams that are closely aligned with disorganized Schizophrenia. More importantly, Adams has symptoms aligned with limited attention span whereby Adams performs dismally in his studies contrary to the past whereby he was an exemplary student. The notion that Adams has limited attention span is the fact that he has had poor school attendance. Many at times, persons with disorganized Schizophrenia cannot concentrate in the activities they engage in as a result of limited attention span. On a similar note, restlessness and irritability is the other pertinent symptom that confirms Axis 1 diagnosis of Schizophrenia in Adams case.

Axis 1 diagnosis of Schizophrenia as in Adams case is further justified by altered thought process exhibited by Adam. Adam has delusional thinking, which is apparent during the consultations whereby he gives incoherent responses for the questions asked. Another symptom of altered thought process depicted by Adam is the fact that he shifts from one topic to another. This is evident when Adam is asked if he knows the reasons for his presence during the clinical visit whereby he responds that it may be because he had never dated and the cow ran with the spoon. In this case, Adams responses deviate from the question of focus to the other. On another note, Adam is not able to stay focused on various matters. Despite the fact that Adam probably has injuries in his feet, but is yet to seek help on the same. In fact, it seems Adam has not realized the excoriations in his skin.

Social isolation is the other rational that justifies the axis 1 diagnosis of disorganized Schizophrenia in Adams case. Social isolation is apparent in Adams case because he has limited or no interactions with others. Adam spent all the summer in his room and also refused to attend his familys vacation and stayed at home alone. In a nutshell, Adam seems to be avoiding social situations, which entail interaction with others. Besides, Adam seems depressed as evident by the fact that his hair is unkempt, his clothes are dirty, and he is disheveled. It is also of the essence to realize that axis 1 diagnosis of disorganized Schizophrenia in the case of Adam is because of the alterations he manifests that are closely related with hallucinations. Despite being unable to concentrate, Adam provides unsuitable reactions and responses that are not real.
